CLTS Services
Children’s Long-Term Support Services
The Children’s Long-Term Support (CLTS) Waiver Program is a Wisconsin Medicaid program that helps children with disabilities and their families access the support they need to thrive at home, in school, and in the community.
Through CLTS, families can receive flexible, individualized services that build skills, increase independence, and make daily life more manageable. Unlike therapy covered by health insurance, CLTS services focus on supporting children and caregivers in everyday routines, community participation, and safety.
The Center for Behavior Change is a Wisconsin CLTS provider statewide for participants of all ages. Services outside of Dane County are offered remotely.

This service provides training, education, and support to unpaid family members or other caregivers who provide daily care for a child with a disability.

To support the child/youth and family in identifying, preparing for, and preventing safety risks in home and community settings.

Services are designed to support the child in communicating effectively in community life (stores, libraries, playgrounds, rec programs, etc.)

Services meant to support the child/youth in learning and practicing everyday skills that foster independence at home and in the community. Focus is on functional skill-building in natural environments.

To support the child/youth in building social connections and participating in inclusive community activities (clubs, sports, arts, recreation, volunteering). CIS emphasizes natural participation and integration, not clinical therapy goals. The focus is on reducing isolation, building confidence, and accessing typical childhood experiences.

Personal Supports are meant to give a child direct assistance to participate in daily routines and community life. Remote delivery works by actively guiding the child in real time (with the caregiver acting as your hands when needed).

Caregiver Support
This service is designed to support parents and caregivers in gaining knowledge, confidence, and practical tools to help their child thrive. A BCBA provides individualized training on topics related to your child’s disability, development, and behavior. This servic is tailored to your family’s needs and may include understanding your child’s diagnosis, fostering communication and independence, managing daily routines, or preventing and responding to challenging behaviors.

Functional Behavior Assessments
This service focuses on understanding the reasons behind challenging behaviors and creating a clear, compassionate plan for support. A BCBA works with your family and team to conduct a Functional Behavior Assessment (FBA) — a structured process for identifying what your child is communicating through their behavior.
This process empowers families and caregivers to understand why behaviors occur, respond with consistency, and create a calmer, more predictable environment for learning and growth.

Foundations Program
This service is designed for children and families who are new to a diagnosis and ready to begin building skills for a successful future of learning. During this stage, a Board Certified Behavior Analyst (BCBA) works closely with your family to identify strengths, priorities, and areas for growth.
The focus is on foundational skills that support later learning and independence, such as: Early communication and social engagement, emotional regulation and coping strategies, participating in daily living routines, transitioning from activities, play and early problem-solving skills, and family education.
